Steps to reintroduce:
1. go to a level which has both elevators and cacodemons. Map05 is okay.
2. Maneuver a cacodemon so that it's on top of the elevator
3. Lower the elevator
4. Step below the cacodemon and wait
You will be stuck inside the cacodemon,

Hmm, had to create a test map for this one, I tried doing the test as you described here, but it was too tedious, it seems to have improved with the latest commit, let me know if it fails.
